One of the Department of Education’s yearly traditions is to send out a survey for the parents/guardians/families. With this ANONYMOUS survey, the DOE wishes to hear your opinion regarding the community the school has built for the students and families, while also allowing to plan for the upcoming school year. Please be aware no one at the school level or the DOE can see your individual responses and cannot tie your access codes to survey results.

1. Families will enter “f” followed by their child’s nine-digit Student Identification (OSIS) Number to access the survey. Note that “f” must be lowercase.
2. A student’s OSIS number can be found on a report card, student ID card, or in the family’s NYCSA account
